Entries of Optiworld eligible patients will be made automatically from Optiworld database and entered for the drawing. There is no requirement of any purchase. Eligibility of patients is based on the following:
- Patient must be an Optiworld Medical & Vision Center patient in good standing.
- Patient must be 19 years of age on January 1st of the earliest year of drawing years. In this drawing, the earliest year is 2022.
- Patient must have used services at Optiworld facility consecutively in 2022, 2023 and 2024.
- Patient must have used at least any three (3) of the Eleven (11) services in each of the drawing years (2022, 2023 and 2024).
- The qualifying services are denoted by service codes as (92004, 92014, 92015, 99203, 99204, 99213, 92310, 92072, 92340, 92341, 92342)
These services codes cover vision exams, medical eye exam, contact lenses fitting, scleral lenses fitting, and optical dispensing services for both new and existing patients.
