2025 Optiworld’s Loyal Vacation Patient (LVP) Prize Award

In appreciation of our loyal patients and the relationship we have managed together in the last 30 years (1995 – 2025), Optiworld Vision Center, LLC will hold its second annual Loyal Vacation Patient (LVP) prize award on January 22, 2026, at 5:30 PM.

Eligibility

Entries of Optiworld eligible patients will be made automatically from Optiworld database and entered for the drawing. There is no requirement of any purchase. Eligibility of patients is based on the following:

  • Patient must be an Optiworld Medical & Vision Center patient in good standing.
  • Patient must be 19 years of age on January 1st of the earliest year of drawing years (2023).
  • Patient must have used services at Optiworld facility consecutively in 2023, 2024, and 2025.
  • Patient must have used at least three (3) of the eleven (11) services in each of the drawing years.

Qualifying service codes: 92004, 92014, 92015, 99203, 99204, 99213, 92072, 92340, 92341, 92342, and 92310*.

Drawing

The drawing will take place at Optiworld Vision Center, 2851 Candler Rd., Decatur GA 30034, at 5:30 PM on Jan 22, 2026.

Three (3) winners will be randomly selected, and six runners-up will be chosen as backups.

Prizes

  • 5 days and 4 nights stay in a one-bedroom suite at the Marriott’s Monarch Hilton Head Vacation Resort.
  • $200 vacation stipend for each winner.
  • Optiworld LVP (Loyal Vacation Patient) face cap.

Award of Prizes

  • Winners and runners-up will be notified via email within 24 hours of the drawing.
  • Winners must accept the prize within one week.
  • Reservations must be made before deadlines to qualify for the $200 stipend.
  • Vacation period: June 15 to August 16, 2026.

Rules

  • Only one entry per person.
  • No purchase necessary.
  • Optiworld staff and their families are not eligible.
  • Participants may opt out by notifying Optiworld before the drawing date.
  • Winners agree to the publicity of their names and photos.

Terms & Conditions

Optiworld is not responsible for the condition, service, or suitability of the resort. Winners are responsible for additional costs such as travel, entertainment, and any property damages incurred at the vacation destination.

Limitation of Liability & Indemnity

Use of the vacation resort is at the winner’s own risk. Optiworld Vision Center, LLC, and its affiliates shall not be liable for any direct or indirect damages resulting from the use of the prize.

Termination

Winners or Optiworld may terminate the use of the prize at any time before check-in.
