LVP CONTEST DETAILS
ELIGIBLITY PRIZES RULES
Entries of Optiworld eligible patients will be made automatically from Optiworld database and entered for the drawing. Eligibility of patients is based on the following:
- Patient must be an Optiworld Medical & Vision Center patient in good standing.
- Patient must be 19 years of age on January 1st of the earliest year of the tree (3) eligibility years.
- In every annual eligibility contest, the earliest year is the first year of the three (3) consecutive eligibility years.
- Patient must have used at least three (3) of the eleven (11) services in each of the eligibility years as part of their eye care services at Optiworld Medical & Vision Center.
- The qualifying services are denoted by service codes as (92004, 92014, 92015*, 99203, 99204, 99213, 92310*, 92072*, 92340*, 92341*, 92342*)
- Procedure codes with * marking are professional service codes with affiliate codes.
- Affiliate codes are codes used in eyewear purchases – glasses, contact lenses, and accessories.
- For affiliate codes involving contact lens materials, only the annual supply of materials will be eligible.
- The requirement of each procedure code with an affiliate code (*) must be met in order to use the procedure code as part of the LVP eligibility verification process.
- Only Optiworld management can modify or exempt any part of these rules and eligibility requirements.
